Vermont health center gets national recognition

PLAINFIELD (AP) - The Health Center in Plainfield has been recognized for special designation by a national health care accrediting group.

The clinic on U.S. Route 2, which as a federally qualified health center is able to offer a sliding scale discount to eligible patients, got the Level Two Patient Centered Medical Home designation from the National Committee for Quality Assurance.

It means The Health Center is recognized for high-quality care and maintenance of good health in the communities it serves.

The concept of a patient-centered medical home focuses on teamwork, quality care, chronic disease management and prevention. Instead of focusing on a patient's treatment only when they are sick or injured, the medical home seeks to deliver on-going preventative and holistic care.
